Any non physical intentional repeated work related behavior between persons with or without difference of power that has adverse effects on employee’s emotional and mental well-being in the working place.

Effects of emotional abuse

Emotional abuse among employee is very alarming signs of productivities of organization.   • Talk to the abuser directly with confidence and a rational attitude rather than taking to someone about the situation. Make them clear that you are not going to bear it and if it will persist then will take the help of higher authority.
  • Even after proper communication and warning, the abuser doing this persistently, then it is high time to report the matter to higher authority. Matter should be escalated though proper documentation and proper channel. Collect your evidence and witness, if any, to produce in front of authority. Don’t try to make false allegation, it will make you weak and will decrease your confidence to confrontation.
  • Keep yourself strong and confident. Discuss your problems with spouse, family, friends with you have trust. Official gossips will, make you weak and you will feel harassed.

I am not biased, I usually ask my patient about their sexual life who come for psychiatric consultation.

I observed in my clinical experience if couple therapy, the need of sex in marriage is not a universal phenomenon. Some of them accept it as very much necessary and utmost important while others say this is just a physical need.  The opinion of people varies as also explained in Nancy’s thought. So one opinion does not fit for all and varies from couple to couple.I have come across a few married couples who get separated because of disturbed sexual life while others were leading a happy married life without sex for last 10-15 years. Some of the couple is date for years without sexual activities, few postpone it till marriage. So everyone has different opinion and it ultimately depends on their personal belief, desire, nature of the relationship and mutual agreement.  Many people have happy, fulfilling, romantic, stable, and joyful married life without having sex. So we can understand sex is not everything required for a satisfied and healthy marital relationship.

Why sex is important?

There is nothing wrong if one partner demands regular sex from other or believes this is a vital part of their relationship. Some people don’t feel emotionally connected if their sexual demands are not fulfilled. They feel that sex is a way to express their love; care and affection to partner .Few also think that having regular sex is a sign of loyalty. Person with high libido believe sex is a sign of love. Reasons for frequent and excessive desire of sex could be insecurity with partner, part of their fun or entertainment, boredom relief, assuage skin hunger, too much lust and many more often too complicated to define and understand.

Libido or sexual desire

It’s rare for two people to have matching sex drives. It happens occasionally, but to expect that your partner will always want precisely the same amount of sex as you do is mostly impractical.

Obsessive-compulsive disorder

Obsessive-compulsive disorder (OCD) is represented by a diverse group of symptoms that include intrusive thoughts, rituals, preoccupations, and compulsions. These recurrent obsessions or compulsions cause severe distress to the person. The obsessions or compulsions are time-consuming and interfere significantly with the person’s normal routine, occupational functioning, usual social activities, or relationships. A patient with OCD may have an obsession, a compulsion, or both. In contrast to an obsession, which is a mental event, a compulsion is a behavior. Specifically, a compulsion is a conscious, standardized, recurrent behavior, such as counting, checking, or avoiding. A patient with OCD realizes the irrationality of the obsession and experiences both the obsession and the compulsion as unwanted behavior. Although the compulsive act may be carried out in an attempt to reduce the anxiety associated with the obsession, it does not always succeed in doing so. The completion of the compulsive act may not affect the anxiety, and it may even increase the anxiety. Anxiety is also increased when a person resists carrying out a compulsion.

Prevalence

Among adults, men and women are equally likely to be affected, but among adolescents, boys are more commonly affected than girls. Early twenty is the onset of illness in both gender.

How to recognize? Symptoms of OCD?

Person feels thoughts and anxiety, most days for at least 2 successive weeks and be a source of distress and/ or interference with usual activities.

  1. They have recurrent and persistent thoughts, urges, or images that are experienced, at some time during the disturbance, as intrusive and unwanted, and that in most individuals cause marked anxiety or distress.

2 . The individual attempts to ignore or suppress such thoughts, urges, or images, or to neutralize them with some other thought or action by performing a compulsion. Compulsions are repetitive behaviors (e.g., hand washing, ordering, checking) or mental acts (e.g., praying, counting, repeating words silently)

OCD comes generally 4 categories:

  • Checking,
  • Contamination,
  • Symmetry and ordering,
  • Ruminations and intrusive thoughts, .

Examples of obsession include:

  • Fear of being contaminated by touching objects others have touched
  • Doubts that you’ve locked the door or turned off the stove and having difficulty tolerating uncertainty
  • Intense stress when objects aren’t orderly or facing a certain way
  • Images of driving your car into a crowd of people
  • Thoughts about shouting obscenities or acting inappropriately in public
  • Unpleasant sexual, religious images
  • Avoidance of situations that can trigger obsessions, such as shaking hands

Examples of compulsion include:

  • Hand-washing until your skin becomes raw
  • Checking doors repeatedly to make sure they’re locked
  • Checking the stove repeatedly to make sure it’s off
  • Counting in certain patterns
  • Silently repeating a prayer, word or phrase
  • Following a strict routine
  • Arranging your canned goods to face the same way
  • Demanding reassurance

 

The types of obsessions and compulsions you experience can also change over time. Symptoms generally worsen when you experience greater stress.

OCD, usually considered as a chronic and  relapsing and remitting course of illness, can have mild to moderate symptoms or be so severe and time-consuming that it becomes disabling.

 

Complications if untreated

Problems resulting from obsessive-compulsive disorder may include,

  • Excessive time spent engaging in ritualistic behaviors.
  • Health issues, frequent hand-washing may lead to contact dermatitis.
  • Frequent absenteeism job, school or difficulties to attend social function.
  • Poor academic, work, job performance.
  • Relationship problems.
  • Overall poor quality of life.
  • Suicidal thoughts and behavior.

Pharmacotherapy, behavior therapy, or a combination of both is effective in significantly reducing the symptoms of patients with OCD.

The decision about which therapy to use is based on the clinician’s judgment and experience and the patient’s acceptance of the various modalities. About 8 to 1 6 weeks are usually needed to obtain maximal therapeutic benefit.

Pharmacotherapy:

Few medicines are very effective to reduce the irrational thoughts, anxiety and improve quality of life. Ant anxiety, anti depressants is very useful. SSRI, SNRI, TCA Clomipramine, Benzodiazepines are very helpful medicine. Repetitive Trans-magnetic Stimulation (rTMS) is a modern therapy. Very rarely neuro-surgery is also helpful.

Psychotherapy: Many clinicians consider psychotherapy is treatment of choice for OCD. Its very effective treatment along with medicines.

  • Behavior therapy – Desensitization, thought stopping, flooding, implosion therapy, and aversive conditioning have also been used in patients with OCD.

Insomnia

Insomnia is subjective perception of difficulty with sleep initiation, duration, maintenance, quality, or non refreshing sleep, despite adequate opportunity or situation for sleep. It must be present for at least 3 nights per week for at least 3 months, and not be related to other sleep, medical &surgical or mental disorders. Studies showed the prevalence of insomnia is 10%–30% of the adult population. It is common in older adults, females, and people with medical and mental ill health. Sleep serves a restorative, homeostatic function and appears to be crucial for normal thermoregulation and energy conservation. As NREM sleep increases after exercise and starvation, this stage may be associated with satisfying metabolic needs.

10-15% of adult people experience long-standing, 25-30% has occasional sleepless nights.

It is an easy-to-diagnose condition but still unrecognized in a significant number of patients attending outpatient department (OPD) for other medical conditions.

What Are REM and Non-REM Sleep?

A sleep cycle lasts about 90 minutes, and during that time we move through five stages of sleep. The first four stages make up our non-rapid eye movement (NREM) sleep, and the fifth stage is when rapid eye movement (REM) sleep occurs.

NREM (75 percent)

Stage 1 : 5 %, eyes are closed, but it’s easy to wake you up, very light sleep, transition from wakefulness to sleep, last 5 -10 minutes.

Stage 2: 45 %, body temperature drops and heart rate begins to slow, body is getting ready for deep sleep, lasts about 20 minutes.

Stage 3 : 12%, deeper sleep, arousal,  body muscles relax, decrease of heart rate and blood pressure.

Stage 4: 13%, most of stage 4 sleep occurs in the first third of the night, deepest sleep, difficult to arouse, if someone woke you up, you would feel disoriented for a few minutes.

REM (25 %)

Most REM periods occur in the last third of the night, brain becomes more active, body become relaxed and immobilized, and dream occurs, eyes move rapidly.

During the deep stages of NREM sleep, the body repairs and re-grows tissues, builds bone and muscle, and strengthens the immune system. With age NREM sleep get less, and REM sleep duration increases.

MEDICAL TREATMENT:

Detailed history of insomnia is the first step of diagnosis. Investigation like polysomnography (sleep study) should be done if required. Anti anxiety, anti depressants, melatonin supplements, sleep inducing medicines, are helpful. Treatment of underlying health issues, allergy or cold medication, use of breathing device or surgery (usually for sleep apnea), a dental guard (usually for teeth grinding) is useful.

SLEEP HYGIENE TIPS

 1) Fix sleep time. This is the best ways to train body and mind to sleep well, even on weekends and days off! Go to sleep only when feel sleepy, or tired, rather than spending too much time awake in bed.

2) Get up & try again. If you haven’t been able to get to sleep after about 20 minutes or more, get up and do something calming until you feel sleepy, then return to bed and try again. Sit quietly on the couch with the lights off, read something. Avoid doing anything that is too stimulating or interesting, as this will make you alert and wake you up even more.

3) Avoid caffeine & nicotine. It is best to avoid consuming any caffeine (in coffee, tea, cola drinks, chocolate, and some medications) or nicotine (cigarettes) for at least 4-6 hours before going to bed. These substances act as stimulants and interfere with the ability to fall asleep

4) Avoid alcohol. Many people believe that alcohol is relaxing and helps them to get to sleep at first, but it actually interrupts the quality of sleep.

5) Bed is only for sleeping. Don’t try to use your bed for anything other than sleeping and sex, so that your body comes to associate bed with sleep. Don’t use to watch TV, eat; read, work on your laptop, and other things.

6) No day time naps. It is best to avoid taking naps during the day.

7) Sleep rituals. You can develop your own rituals of things to remind your body that it is sleeping time.

 8) Bath time. Having a hot bath 1-2 hours before bedtime can be useful, as it will raise body temperature, making sleepy.

 9) No repetitive clock-watching. Many people who struggle with sleep tend to watch the clock too much. Frequently checking the clock during the night can induce negative thoughts makes you alert and wakeful. Thoughts are like  “Oh no, look how late it is, I’ll never get to sleep” or “it’s so early, I have only slept for 5 hours, this is terrible” making them more difficult to sleep.

10) Use a sleep diary. This worksheet remains very useful way of making sure you have the right facts about the sleep, rather than making assumptions. it also helps to track the improvement.

11) Exercise. Regular and light exercise is a good idea for good sleep. Morning walks are a great way to start the day feeling refreshed.

12) Eat right. A healthy, balanced diet will help to sleep well, but timing is important. Too light and too heavy meal is not good for sleep. A warm glass of milk, which contains tryptophan, acts as a natural sleep inducer. Avoid fat and spice rich meal.

13) The right space and surroundings. It is very important that bedroom should quiet and comfortable for sleeping.

14) Keep daytime routine the same. Even if you have a bad night sleep and are tired it is important that you try to keep your daytime activities the same as you had planned. That is, don’t avoid activities because you feel tired. This can reinforce the insomnia.

About depression

Depression is a mood disorder, and shows persistent symptoms of low mood for 2 weeks affecting person’s daily activities. It affects all age group and gender. But there are differences in presentation, severity and prognosis of illness. 20-40 age groups are most vulnerable age group, highest among individuals between 18 and 25, but it doesn’t spare child and adolescent’s age group and elderly population. It affects more female than male. Sometimes it has auto remission of course and wanes its symptoms gradually but many more suffer badly. Depressive disorder may recurrent episode and one can predict it. Yes few vulnerable factors are there those can support to predict the relapse of symptoms. Depressive episode can be a part of bipolar illness. But mostly it occur its own course.

Prevalence

Prevalence of depression ranges from 20% -25% in women and 7% – 12% in men, higher among adult females (9%) compared to males (6%), 9-10% of adolescent age group.

Symptoms

  • Feeling low energy most of the time despite good physical health,
  • Remain in low mood,
  • low interest level in previously pleasurable activities, communication with friends and family, decreased interaction with others, study, work, sexual activities, exercises,

Apart from this patient has, body pain, headache, loss of appetite or increased appetite (in very few person), decreased attention and concentration, problems in taking decision, forgetfulness, anxiety, restlessness.

How is Depression Different from Sadness?

Sadness is temporary condition and associated with some stressors. It goes automatically in days. It does not affect much on other activities. Appetite, sleep, interest remain same. But depression is a persistent low mood and associated with other symptoms.

Possible reasons of depression:

Most of the episode not associated with any kind of stressors. but Relationship problems, financial loss, job loss or dissatisfaction may be reasons.

Medical illness can cause depression like postpartum, hypothyroidism, diabetes mellitus, hypertension, cancer, few neurological illnesses, and many more. There are few Biochemical changes like low serotonin, low dopamine level, increased cortisol level, is usually seen.

if remain untreated affects personal, family, professional domain of life like Marriage life,  relationship problems, friendship, job, finances, decreased performances, suicide are also common.

People think medicines are very addictive and usually have hypnotics, how medicine or counseling will help? They also think depression is only a mental thought, will be fine automatically, go and tour, eat good, do party.

Depression is treatable, with talking therapies or antidepressant medication or a combination of these. It reduces suffering, duration of illness, and prevents relapses in future. • Generalized anxiety disorder includes persistent and excessive anxiety and worry about general ordinary or routine activities or events.
• Panic disorder is repeated episodes of sudden feelings of intense anxiety and fear or terror that reach a peak within minutes (panic attacks), characterized by feelings of impending doom, shortness of breath, chest pain, increased heart beats (palpitations). These situations may result in constant fear or worrying about them happening in future and that they start avoiding such situations during which they’ve occurred.
• Obsessive Compulsive Disorder is characterized by Obsessions which are recurrent thoughts, ideas, images or impulses that are senseless, involuntary, unwanted and distressful and causing anxiety; and Compulsions which are repetitive mental or physical acts or rituals that are repetitive in nature and are performed to reduce the anxieties caused by the obsessions.
• Social anxiety disorder (social phobia) is high levels of tension, fear and avoidance behavior of social situations because of feelings of embarrassment, self-consciousness, self-criticism and concern about being judged or viewed negatively by others.
• Specific phobias are characterized by major anxiety when you’re exposed to a particular object or situation and a desire to avoid it. Phobias provoke panic attacks in some people.
